Form 5 Information:

What you need to do before you can take your Form 5 “ride”:


  1. Assure CAP level 1 training is recorded at National CAP.

  2. Complete the annual CAPR 60 – 1 test on the CAP website and print out the completion certificate (log onto CAP eServices, https://www.capnhq.gov select CAP Online Exams from the left column. On the Professional Development Page, scroll down to Standardization & Evaluation and click on “Form 5 Annual Examination-Powered”).

  3. Complete the CAP Orientation Pilot test if applicable (recommend without ROTC version initially) by going to the same page as in item 2 above and select “Orientation Pilot Exam – Powered”. Remember to print out the completion certificate.

  4. Complete the annual Form 5 aircraft questionnaire for the plane you will be flying for this flight. Make sure you obtain the current aircraft data. Note: you need to do a questionnaire for any aircraft you have previously been checked out in and wish to remain current in as well.

  5. Bring the above, a CAPF5 (available on CAP eServices under “CAP Pubs & Forms”) with your personal information filled in, your logbook, CAP membership card, FAA License & medical, etc (see 60-1) to the ride appointment.

  6. Assure copies of above items, and all items such as Statement of Understanding, etc, as listed in CAPR 60-1, paragraph 2-8, are contained in your personnel file. Add to this list a photocopy of your most recent logbook page to substantiate experience.


Once you have completed your Form 5 “ride”:


  1. Submit the CAPF5 to the Squadron Ops. Officer for approval.

  2. Once approved it must be entered into the MIMS and approved by the Wing D.O. before you can act as PIC of a CAP aircraft!

  3. To enter Pilot info into the MIMS: Log into the CAP eServices site at https://www.capnhq.gov

  4. On the lower left hand side click on “My Operations Qualifications/National Reports”.

  5. Then at the top of the page, click on “Pilot”. The linked page will open.

  6. On the lower left, click on “FAA Requirements” and update/enter the data and click submit (or use back if no updating occurred).

  7. On lower left, mouse point on “Check Ride Questionnaires” and select (to the right) “airplane” and click on it.

  8. You will now see your check ride/currency page

  9. Scroll down half way and you will see your latest 60-1 date, if not there, enter it.

  10. Just below it, enter the date of the grading of the aircraft questionnaire.

  11. Just below that select the aircraft type of the questionnaire from the list (note you can select more than one if multiple questionnaires were submitted at the time of the ride).

  12. In the next column, select whether an initial or annual check ride. If this is the first time you’re entering a CAPF5 in MIMS you must check the annual checkbox. You must check initial if this was the first time in this type of aircraft regardless of any previous check rides (i.e. if you did a C182 ride four months ago and this was a C172 ride you MUST check initial for the C172 ride as it is the first time you are being checked in this aircraft type).

  13. Just below, select any endorsements that were checked on the CAPF5 form (note that any previously completed endorsements will be shown in green on the lower right).

  14. Check all the entered information for accuracy and click submit on the bottom of the page once. After several seconds it will confirm the submission and tell you it is awaiting approval by the Wing D.O.

  15. Endorsements etc. will show on your MIMS records in Gold (“pending”). They will change to green (approved/active) and/or be listed on the top of the “pilot” page usually within 48 hours.

  16. While on the page, check your CAP Aeronautical Rating by clicking on “CAPR 35-6 Aeronautical Rating” on the lower left of the page. If you have an FAA Pilot Certificate and less than 1,000 hours you have a CAP Pilot Rating, if you have between 1,000 and 1,999 hours you are a CAP Senior Pilot, and more than 2,000 hours you are a CAP Command Pilot.

  17. Once all your Form 5 info has been properly approved, you can use the “What Do I Need” option on the lower left to see what you need to reach a certain achievement.

  18. Reminder: Make sure copies of the “approved” Form 5, aircraft questionnaire(s), 60-1 certificate, current medical, Pilots License, a photocopy of your most recent logbook page to substantiate experience and all 60-1 paragraph 2-8 items are in your Squadron file!


CAP Pilot Qualifications for Powered Aircraft*


Solo Pilot = 16 yrs old + meet all FAA Requirements

Pilot = 17 yrs old + Pvt. or higher license + Class III Medical

COP (non AFROTC) = 200 hrs PIC time, 21 yrs old

COP (AFROTC) = 300 hrs PIC + Comm. Cert. + Class II Med, 21 yrs old

TMP = 100 hrs PIC including 50 hrs xcntry + qualified GES, 18 yrs old


To fly CAP “High Performance” Aircraft*


100 hours pilot experience

Successfully complete CAP transition flight training program

Successfully complete CAPF 5 Proficiency Check Ride per 60-1

Meet all FAA requirements and have the proper logbook endorsement for PIC in High Performance Aircraft.
